Who is CareCentrix?
CareCentrix is an expert in home care coordination. Our Post-Acute Care program provides personalized care plans with an emphasis on recovery at home. We work with network providers – including hospitals, post-acute care facilities, and home care providers – to identify paths for care after hospitalization and coordinate services to ensure your safe recovery.
When you are home, we connect you with Registered Nurses through our Nurse Coaching Program. Our Nurse Coaches meet with you over the phone to provide support for your recovery plan to meet your needs and goals. Throughout your recovery, our Nurse Coaches are here to answer any questions you, your family members, or caregivers may have.

Your Nurse Coach can help you understand your discharge instructions, review your medications, and help answer any questions you may have to support your recovery. You can also ask about the resources in your own community if you need support with transportation, meals or medication assistance.

Yes, your Nurse Coach can help you find the resources and support you need along your recovery. This can include transportation services, meals support, financial assistance, and medication assistance.
What are the costs?
If eligible, our program is covered under your health plan.
Does my Nurse Coach visit me in person?
No, this is a telephonic program. Your Nurse Coach will call you to discuss your recovery plan to help you meet your goals.
How long will I be in contact with my Nurse Coach?
Everyone’s recovery is different. The Nurse Coach will evaluate your progress based on your personalized recovery plan during each discussion.
