CareCentrix, an industry leader in making the home the center of patient care, today announced the launch of HomeBridge, a proprietary cloud-based platform that enables patients to heal and age in their homes, and avoid unnecessary hospitalizations and readmissions. Built on Salesforce Health Cloud, HomeBridge connects patients to the providers and care they need on their paths home. CareCentrix developed HomeBridge to provide a complete view of a patient’s data, track clinical events over time, and collaborate to provide exceptional care. Health Cloud’s powerful engagement features also allow CareCentrix to connect patients and care teams to facilitate continuity of care as patients heal at home. HomeBridge delivers new accountability and access to care in the home that leads to lower costs and higher patient satisfaction.

Home is the unknown, underutilized setting of care that can help appropriately accelerate the transition from high-cost fee-for-service models to value-based care. Not only do patients prefer to heal or age in the comfort of their home, but doing so also safely and effectively reduces the reliance on higher-cost settings, such as hospitals or skilled nursing facilities. The Centers for Medicare and Medicaid Services (CMS) and legislators are increasingly focused on care in the home, as evidenced by the Comprehensive Care for Joint Replacement model and proposed mandatory bundled payments for cardiac care.

According to CareCentrix data, 69% of patients leaving the hospital have a new prescription at the time of discharge, but just above half of those on a new medication understand the potential side effects of it. Nearly a quarter of discharged patients are suffering from depression, and 25% don’t have a follow-up appointment with their doctor. For a patient with significant health conditions, such as chronic obstructive pulmonary disease (COPD) and congestive heart failure, these statistics potentially translate into dangerous gaps in care after a hospital stay.

With the real-time, continuous HomeBridge connection between patients, providers, centralized nurse teams, telehealth providers and community resources, the patient’s entire care team will be able to see a daily record of information like blood pressure and heart rate and intervene when needed. The HomeBridge platform will be presented live by John Driscoll and Christina Mainelli at the Dreamforce 2016 conference taking place in San Francisco, Oct. 4-7.
